The first thing you need is access to some kind of broadband service in the location where you are planning on making your calls. More specifically, you will need a wired connection like DSL or cable and a wireless extension. If you do not want to use a computer to make calls (remember, computer-to-computer calls will tend to be free, while those made to and from actual phones generally cost money), you will then need to connect the base of your phone to the wireless router. You can use either a cordless phone or one with a cord. In addition to your broadband service, you will also need to sign up for service with an Internet phone provider. You will have a number of companies to choose from, and your selection should depend on how you plan on using the service.
If you are going to be using a computer to make your mobile broadband phone calls, then you will either need a computer with a built-in microphone and speakers, or you will need to obtain an external microphone and headphone set. Of course, if you are going to use a regular phone, then you will not need these extra devices. If so, using VoIP will feel a lot like using a land line, but instead of plugging the phone into a phone jack, you will plug it into the wireless router. You will also have access to features like caller ID, call waiting, and conference calling, which are all pretty common among land lines these days.
